The Consumer Product Safety Commission reissued a 2008 recall of more than 985,000 drop-side cribs by Delta Enterprise Corp. because of the death of a 7-month-old girl from Colorado Springs, Colo. The cribs were sold at major retail stores including Kmart and Target between January 1995 and December 2005.
WHY: Missing safety pegs can create a situation where the crib's drop-side rail disengages from the track, creating a dangerous gap where an infant can become entrapped and suffocate.
INCIDENTS: At the time of the 2008 recall, CPSC notified consumers about the death of an 8-month-old girl who suffocated when the drop-side of the crib detached. The agency learned earlier this year of another death: that of the 7-month-old girl from Colorado Springs, who became entrapped and suffocated between the detached drop-side and mattress of her recalled crib. Her crib was purchased secondhand and re-assembled without safety pegs in the bottom tracks, according to CPSC.

DIVING MASKS
DETAILS: Guardian full-face diving masks made by Undersea Systems International Inc. doing business as Ocean Technology Systems, of Santa Ana, Calif., and sold at diving equipment retailers and direct sales nationwide from September 2010 through November 2010.
WHY: The purge assembly on the diving mask can disengage from the regulator, resulting in loss of air to the diver.
INCIDENTS: No injuries but the company has received one report of a disengaged assembly.

TOILET BOWL CLEANER
DETAILS: Instant Power Toilet Bowl Restorer cleaner distributed by Scotch Corporation, of Dallas, and sold at Menards, True Value Hardware, Ace Hardware, and other retail stores nationwide between February 2009 and January 2010.
WHY: The contents can leak from the cap when the bottle is turned on its side. When this happens, the cleaner can come into contact with consumers and property, posing a risk of chemical burns and irritation to the skin and eyes.
INCIDENTS: No injuries, but Scotch has received seven reports of bottles leaking, resulting in property damage.

LEBANON BOLOGNA
DETAILS: Lebanon bologna, made in December 2010 by Palmyra Bologna Co. Inc. of Palmyra, Pa., and sent to centers in California, Colorado, Maryland, New York and Pennsylvania for further distribution to retail stores.
WHY: E. coli contamination.
INCIDENTS: Four cases in New Jersey, North Carolina, and Pennsylvania of illness that began between Jan. 28, 2011, and Feb. 12, 2011.
